Trove Health Inc. ("Trove Health," "we," "us," or "our") is committed to transparency and to honoring the privacy rights of all Individuals who use our Individual Access Services (IAS). In accordance with the IAS Provider Requirements Standard Operating Procedure (SOP) and applicable law — including the Health Insurance Portability and Accountability Act (HIPAA) and the Trusted Exchange Framework and Common Agreement (TEFCA) — you have the right to revoke your consent to the terms of Trove Health's Privacy and Security Notice at any time.

2. Your Right to Revoke Consent

You may revoke your consent to Trove Health's Privacy and Security Notice at any time, without penalty, by following the procedure set forth below. Revocation of consent is effective upon processing and will result in Trove Health ceasing to access, exchange, use, or disclose your Individually Identifiable Information (III), except as required by applicable law.

⚠️ Important Notice: Revocation of consent may limit or permanently end your ability to access and use Trove Health's Individual Access Services, including the retrieval of your health information via TEFCA Exchange.

3. Step-by-Step Consent Revocation Instructions

To revoke your consent, please follow the steps below precisely:

1. Prepare Your Request
  
Gather the following information before submitting your revocation request:
    -- Your full legal name (as registered with Trove Health)
   -- Your registered mobile number associated with your Trove Health account
   -- A clear, written statement expressing your intent to revoke consent

2. Submit Your Revocation Request
   Send an email to Trove Health's Privacy Office at the address below. Ensure that your email contains all three items listed in Step 1.    Incomplete requests may delay processing.

3. Await Confirmation of Receipt
    Trove Health will acknowledge receipt of your revocation request within two (2) business days of receiving your email. If you do not     receive a confirmation within this period, please follow up at privacy@trovehealth.io or call +1 (415) 800-4442.

4. Processing of Revocation
    Trove Health will fully process your revocation request within five (5) business days of confirming receipt. You will be notified once     processing is complete.

4. Effect of Revocation

Upon full processing of your revocation request, the following will take effect:As an Individual using Trove Health's IAS, you retain the following rights in connection with TEFCA Exchange:

-- Trove Health will cease accessing, exchanging, using, or disclosing your Individually Identifiable Information in connection with     Individual Access Services.
-- Your ability to use Trove Health's IAS, including TEFCA Exchange, may be limited or terminated.
-- Trove Health will continue to retain certain information as required by applicable law, including information contained in audit logs,     which is not subject to deletion.
-- Revocation of consent does not affect the lawfulness of any processing that occurred prior to the revocation.

5. Alternative Options

In addition to full consent revocation, you may also exercise the following rights under Trove Health's Privacy and Security Notice without fully revoking your consent:

-- Opt out of TEFCA Exchange while retaining your account
-- Request deletion of your Individually Identifiable Information
-- Restrict use or disclosure of your information
